This feature is designed to support patient and coach insights related to health measurements and health action trends over time. You may utilize notification systems to keep patients engaged with their self-monitoring. Quality Health Action Plans should be created within 24 hours of enrollment. To find patients without Health Action Plans, go to your "New Enrollments" dashboard.
Here are the steps to create a Health Action Plan. For more information about the Health Action Plan, download the PDF attachment at the bottom of this article.
1. Create a Health Action Plan
You can build a health action card in your patient's account by clicking the new action plan button:
2. Click "Add health measurements and actions":
Click "Add" to select items for the Health Action Plan. You can change or remove individual items or the card itself by clicking "Edit." To further engage your patient, you can personalize the Health Action Card title to reflect your coaching conversations. This title will appear on your patient's progress screen.
ProTip: Keep that title short and meaningful, like what you see below. You can convey the motivation or purpose for that plan in one to two lines!
3. Specify Item Details and Display:
Follow three easy prompts to build individual items for your patient's health plan:
- Select Action
- Set Details
- Display Options
STEP ONE: SELECT ACTION To best support insights over time, quality Action Plans include health measurements (blood sugar, blood pressure, weight, etc.). This helps patients see the relationship between their behaviors and clinical outcome. Try to start with only 1-2 items in the health action card to keep things simple and targeted toward change. Over time, you may add complexity to the plan. Available items for Health Action Plans include:
Health actions (behaviors such as medications, lifestyle changes like diet and exercise, vitamins, dietary supplements).
and Laboratory values that help patients see longer-term impacts of their behaviors:
STEP TWO: SET DETAILS
Personalize your Health Action by writing in a custom title. You can optionally set goals for health actions by clicking +collect additional data. If you know your patient would like to sync specific data via Apple Health or Google Fit, you must program the health action properly at this step for desired data to be collected in Fitbit Plus. For instance, if your patient would like to sync steps you would need to: select Lifestyle Changes, Regular Exercise as an action and then +collect additional data > Steps to properly complete the sync with Apple Health or Google Fit. Once completed and your patients has completed has allowed data to sync from Apple Health or Google Fit, this data will automatically updated into Fitbit Plus data visualizations.
STEP THREE: DISPLAY OPTIONS Select day and week frequency for the goal in Days per week, times per day.
If Scheduling is turned off, this allows you to add an action to a patient's plan that the patient is not scheduled to complete a certain amount of times per week.
Days and Time enables you to select which day(s) and time(s) of the week that this health action is scheduled to be completed. If tracking is turned on, patients will be reminded each day and time you have selected that the action be completed.
Adherence Tracking turns on reminders for patients and tracks their adherence to these health actions and allows them to customize and manage notifications in their application. Patients must enter data corresponding to the day their health action is set in order for adherence to be measured. Additional data entered outside the day associated with the health action will not be included in the patient's adherence trend and overall adherence data.
TIPS FROM SUCCESSFUL COACHES:
- Don't assume you know what the plan should be, even if you read the doctor’s last note.
- Avoid renewing or changing a plan without checking-in with the patient. Use regular plan check-ins or the messenger feature to support plan changes.
