Fitbit Plus describes patients by three main categories:
- Account Status
- Action Plan Status
- Care Team Status
1. Account Status
Fitbit Plus Patient Population
All patients with accounts in the Fitbit Plus Collaborative Care Platform.
Patients who have completed registration of their accounts, including creation of a password.
Active in Past 7 Days
Patients who have accessed or synced data with Fitbit Plus in the past 7 days.
Active in Past 30 Days
Patients who have accessed or synced data with Fitbit Plus in the past 30 days.
2. Action Plan Status
Has Action Plan
Patients who have at least one action plan. Action plans are the core of Fitbit Plus. They consist of a set of health actions (measurements, medications, lifestyle changes, etc.) that are grouped together, typically to address a particular medical condition, wellness objective, or social need.
Patients who are tracking at least one health action. Health actions can be scheduled to support patients in adherence. By default, scheduled health actions have tracking enabled, which allows patients to log adherence and/or corresponding metrics and allows them to receive reminders about their health actions.
Adherence Trend: Positive
Patients who have missed logging at least one tracking health action between 1 to 2 times.
Adherence Trend: Caution
Patients who have missed logging at least one tracking health action between 3 to 6 times.
Adherence Trend: Negative
Patients who have missed logging at least one tracking health action more than 7 times.
3. Care Team Status
Patients who have opted in to collaborate with the care team on their health. It is not necessary to be registered with Fitbit Plus to be enrolled with a care team, as collaboration can happen outside of the Fitbit Plus app via face-to-face visits, text messages, etc., and the care team members can still document care in Fitbit Plus. Patients who have not yet opted in to collaborate with the care team on their health. Not Enrolled patient accounts are both Not Registered and Not Enrolled. Registration for the Fitbit Plus application or Manual Enrollment by a Care Team member are the only actions that change a patient's Not Enrolled status.
Patients who have opted out of collaborating with the care team (before or after enrolling) or are no longer capable of collaborating with the care team (deceased, moved away, unreachable, etc.). Learn more about archiving here.
Other Important Definitions
Average Coaching Time
Average minutes spent per patient per month. You can view this number within individual patient accounts, or, by your care team in the results dashboard.
This phrase may be used in customer success emails to describe your patients in Fitbit Plus, excluding those who have been archived.
Patients may be assigned to group(s) and team member(s) according to their affiliations within your organization. It is possible that patients exist in a group without yet being assigned a Team Member (coach or other provider) in both the Enrolled and Not Enrolled state.
